2021-2022 Final Forms Are Ready To Be Updated!
Our Final Forms are now available to be updated for the 2021-2022 school year! Please make sure you sign into Final Forms, update any and all information necessary (new address, new phone numbers, updated email addresses, updated emergency contact information, etc.), and add your new signature!
- Please make sure you add any activities or athletics that your student may be involved in. It is easier to remove them from a roster if they decide not to participate. We are always able to add them to a roster however, additional forms may need to be signed after adding them.
- Students must also sign their forms after their parent/guardian has completely filled out the forms and signed them. Parents/Guardians & students MUST have their forms completed and physical completed and turned in before participating in practices beginning in August.
- Incoming 7th graders will have to add their email addresses to their profile. Once that is updated, they will get an email confirmation with their Final Forms login information.
